I have just taken out insurance of foreign nationals and want to pay for it via bank transfer. What are the payment details?

For policies arranged online, premiums can only be paid by credit card. When arranging insurance at partner branches, you must pay by bank transfer. Please transfer the amount to this account number 228 450 171/0300, the variable symbol is your contract number, which can be found in the contract header. The constant symbol is 3558.

How shall I prove my insurance to the doctor?

Identification of the insured in the doctor's office can be done through the insurance policy or a plastic card, which you can apply for at the branch of the insurance company or at the licensed partner with whom you have taken out insurance. Some contracted facilities may verify the validity of the policy directly in the insurer's system. The contracted facility will then bill the insurance company for the care provided that is covered by the insurance.

What does 'coverage in transit countries' mean?

Transit means transportation of the insured from the place of his temporary stay to the state of which he is a national or where his stay has been permitted. The transportation of the insured to such a place must not take longer than 24 hours and must be substantiated by a travel document or proof of purchased fuels.

Are there any insurance benefits limits?

Yes, in the case of comprehensive health insurance, the total limit of indemnity is EUR 80 000, with a limit of indemnity per claim of EUR 60 000.  For Necessary and Urgent Care for Foreigners, the total limit of indemnity is EUR 60 000, including necessary dental treatment with a limit of EUR 50 and repatriation costs not exceeding EUR 10 000 for both Necessary and Urgent Care and Comprehensive Health Insurance.

How does the investigation of insured events take place within the health insurance of foreign nationals?

If you have been treated at a Maxima contracted medical facility, this facility will invoice the insurance company directly - you will not be reimbursed for the costs (except for dental treatment), so we recommend that clients only use contracted medical facilities. If you have visited another medical facility and have paid for the treatment in cash, you must send the insurance company: the original proof of payment, the medical report, the bank account number and the completed Application for Reimbursement of Foreigners' Health Insurance form. These documents should then be sent to info@maxima-as.cz or to the address of the insurer: Maxima pojišt'ovna a.s., Liquidation of insurance claims, Italská 1583/24, 120 00 Praha 2 - Vinohrady, or via your intermediary.
