Submitting a claim

Information for clients:

Only in the case of outpatient care for which the client has already paid.

Documentation required for preparing adjustment:

  • a completed claim (foreigner health insurance) HERE
  • medical report
  • original payment receipts
  • the number of the bank account to which compensation should be credited

All documents should be sent to the address of insurance company:

For other cases treated in medical facilities (acute conditions, injuries) EuroCross Assistance Service must be contacted at: +421 232 114 081.

INFORMATION FOR HEALTHCARE FACILITIES:

Invoices for procedures performed should be sent, along with medical reports, to the address: LPU@maxima-as.cz.
